Before You Register!

A completed current physical form is required for all athletes wishing to participate in any organized FCS Athletics sport.

Please click here to visit the sports physical page. Once the form is printed and completed, it can either:
1. be scanned or a picture taken of it and uploaded to the registration form

2. or physically turned into Mrs. Kim at the Secondary Campus school office.

It is very important that the physical be turned in before registration for that division closes. Otherwise, the athlete will not be able to play until a complete current physical has been submitted.
